Lunare Healthcare provides Occupational Therapy services in the home and community. With funding through Short Term Restorative Care (STRC) Programme or through your Home Care Package (HCP), Lunare Healthcare has a strong focus on enabling people to participate as fully as possible in everyday life. We believe that empowering our clients to maintain their independence, mobility and function leads to increased confidence, safety, and more engagement within the community.

Lunare Healthcare is able to assist you to navigate the aged care funding space and provide assessments and reports where necessary with the aim to keep you safe and independent in your own home and community.

We offer a variety of assessment services for elderly clients, including:

  • Aged care assessments

  • Home safety assessments

  • Falls prevention assessment

  • ADL (Activities of Daily Living) assessments

  • Equipment review and modifications

  • Recommendations for all aids and equipment (including powered wheelchairs and scooters)

  • Home modifications – Both minor and major

  • Daily living skills training

  • Applications for disability parking permits

The Team

Lauren Feely
Occupational Therapist & Director

​​

Lunare Healthcare was founded in October 2021 by Lauren Feely, an AHPRA-registered Occupational Therapist with a strong background in aged care.

 

Since graduating from Curtin University in Perth in 2017, Lauren has worked extensively with the adult and elderly population, supporting individuals to maintain independence and improve quality of life.

​

Lauren’s clinical expertise includes home modifications (both minor and complex), equipment assessment and prescription, and home safety assessments. She is passionate about working with adults and older people in the community, with a focus on enabling clients to live safely, confidently, and meaningfully in their own homes.

​

Committed to delivering evidence-based, person-centred care, Lauren regularly participates in professional development through training courses, webinars, conferences, and ongoing mentoring.

​

Outside of work, Lauren enjoys going for walks with her husband, daughter and two dogs, spending time with family and friends, travelling and enjoying a coffee and babyccino with her daughter

Hannah Cohen
Occupational Therapist

​

Hannah is an AHPRA-registered Occupational Therapist with over 12 years of experience across hospital, community, and occupational rehabilitation settings.

​

She holds a Master of Occupational Therapy Practice from La Trobe University, completed in 2013, and has additional qualifications in health promotion, mental health, and workplace training.

​

Hannah has contributed to quality improvement projects for major health and corporate organisations, including Alfred Health, Aurora Health, and Australia Post.

​

She is passionate about supporting individuals to maximise their independence at home and providing meaningful clinical supervision to early-career therapists.

 

Outside of work, Hannah enjoys travelling, exploring different cultures, cooking, attending theatre and comedy shows, playing netball, Zumba dancing, and discovering exceptional coffee.

Hannah Vignone

Occupational Therapist 

Hannah graduated from La Trobe University with a Bachelor of Health Sciences and a Master of Occupational Therapy Practice. She has experience working in aged care across both community and hospital settings, and has developed a strong passion for supporting the older adult population.

​

Hannah provides mobile Occupational Therapy services across North and West Metropolitan Melbourne. Her broad scope of clinical practice includes functional assessments, minor and major home modifications, equipment prescription, and home safety assessments. She is driven by a commitment to helping clients achieve their goals and supporting them to remain safe, independent, and confident in their own homes.

​

As an AHPRA-registered Occupational Therapist, Hannah is actively engaged in ongoing professional development to ensure she delivers evidence-based, best-practice care.

​

Outside of work, Hannah enjoys spending time with friends and family, visiting local cafés, and taking her dog to the park.
